New employees

New employees should be provided with the following documents when they start:

ATO tax file number declaration   (opens in new tab)

ATO Choice of Fund form (PDF, 341KB) (opens in new tab)

MLC MasterKey Business Super Product Disclosure Statement (PDF, 25MB)

Default insurance opt-in form (PDF, 116KB) opens in new tab

Details of new employees who join MLC MasterKey Business Super should be included with their first SG payment. We will then establish an account for them and send them a welcome pack.

Existing employees

Existing employees can join MLC MasterKey Business Super at any time by completing a Choice of Fund (opens in new tab) form.
